I'm assuming some sort of pulley but what would it go with?

Looks like you could put a grinding wheel, or maybe a buffing wheel on the one end.

That's right. It's called a bench grinder mandrel, or a saw arbor heavy duty shaft with 2 pillow blocks

Yup, you could put a saw blade on it and cut your fingers off.
Or, you could put a wire wheel on it and pick wires out of your hide.
Or, you could put a grinding wheel on it and hope the exploding pieces don't break a bone in your face.
All kinds of options.
Actually, it really is a simple pillow block arrangement with a grinding wheel adapter.
Your imagination is the only limit.

Similar to those are still sold for carving glass with a v shaped grinding wheel and for other cold working. Lapidaries and gold/silver smithing buffing lathes are similar.
