Thats a Vandegrift patent wrench. Patent Jan 6,1891 Schultz 546 & 724( wood handle)
I have one I bought without the wedge several years ago. Plan to find a broken one and get a wedge. So far no go. THey do come in at least two sizes. The one on e-bay appears to have (11) on it;mine has (14) cast onit. Have seen them sell at auction but haven't wanted one bad enough to pay the price.
