It happens, but not often, and wouldn't be let out of a modern plant.

The round end is for use on the flare nut after you have stripped it with a wrench, a smaller wrench,slip-joint pliers, water pump pliers, and finally vice-grips. It is then perfectly round and fits in the round end of the wrench.....

........and wouldn't be let out of a modern plant.

Why is that?  More eyes, better eyes, electronics to inspect?
