I wish I could post more photos but I don't have the item.  The back was almost identical to the side shown. 
There was a plate with holes on the other side too.

Sept. 20, 08 for sure, the bottom looks like an 03, but sequentially it would have to be 08.

If you have a patent  number, this site works every time and it's simple to use.
Google "patent fetcher", I've not had it fail yet.
