I can't find a lot of information on this one.  Does anyone have one?

Here's a little info. The patent is 553,362. By Jay K. Sheffy.
Interestingly, yours seems to be a closer match to the patent than the one shown on datamp....

Datamp also does not note CHM as a maker....interesting
