Yes, there are variations in the teeth in the one-off handmade combination screw driver shown below (topmost of two), most notably in the height of the teeth. 

The idea of combining the alligator wrench with a screwdriver was a popular one 115 years ago. The little Roebling wrench, the Teddy Screwdriver, and the handmade tool all make this point. 

Does any American manufacturer offer this combination today?

That Roebling is a Freas Patent, originally by American Saw, which was bought by Roebling when they gave up their wrench business. That is Roebling of Brooklyn Bridge fame.
