I've seen for the first time, and now a second sighting, of a double ended 1/2" drive extension.

My first guess is that it was used for standard 1/2" drive sockets and the ratchet was 1/2" female. Is this a correct guess, or is there another reason.

That is the only reason I am aware of

Sometimes an extension is faster to reverse if its double male instead of a loose drive plug that gets stuck in the end of a standard extension, when you are working with female ratchets.
 
I have pried many a plug loose from sockets and extensions both.
In fact, prying the plug loose occasionally is the only real reason to favor a reversible ratchet at all.

  A female ratchet has much better clearance and bulletproof ratchet action, most times.  I use them a lot, always did.
